The university’s voluntary COVID-19 Vaccine Registry is now live online and open for anyone who has received their final vaccine shot.
By registering your vaccine information, you help our campus better understand the breadth of vaccination rates in our community and to better plan for the fall semester.
It is also your opportunity to win a number of prizes. Anyone who registers by August 15 will be eligible for the Grand Prizes.

Frequently Asked Questions
Who is eligible to participate?
UNL students, faculty and staff can be eligible for the prize drawings. To be eligible to participate in the vaccination registry drawing you must:
- Be an admitted and enrolled UNL undergraduate, graduate or professional degree-seeking student for the Fall 2021 semester; or
- Be employed by UNL at the time of the drawing; and
- Submit valid documentation to UNL using the Vaccine Registry Portal by midnight CDT the day before the drawing.

What is the deadline to submit vaccine documentation to be eligible for the prize drawings?
Drawings will be held weekly beginning the week of July 9, 2021. To participate in such drawings, eligible students, faculty and staff must submit valid proof that they have been fully vaccinated against COVID-19 by 11:59:59 p.m. (CDT) the day before the drawing. Weekly drawings will occur on the following dates in 2021:
- July 9;
- July 16;
- July 23;
- July 30;
- August 6; and
- August 13.
The grand prize drawings will held after August 15, 2021.
How will winners be selected?
UNL will randomly select seven (7) students and one (1) faculty or staff member each week to win a weekly prize. For the grand prize drawing, five (5) students and one (1) faculty or staff member will be chosen at random.

How and when will the winners be notified?
Winners will be notified via email at the email address provided in their drawing entry.

Do I have to have received both doses (of a two-dose vaccine) before I can enter the drawing?
Yes, you must receive the full vaccination protocol required by applicable governing authorities, including, the U.S. Food and Drug Administration, European Union, and World Health Organization.
Do I have to wait two weeks after receiving the final dose, or can I enter right away?
You may enter as soon as you have received the full vaccination protocol required by the applicable governing authorities.
Where can I get the COVID-19 vaccine?
You can find vaccination near you on the Vaccine Finder website.
I was vaccinated, but I lost my vaccine card. How can I obtain another copy for my records?
If you lose your card, you can also access your Nebraska vaccination records through the Nebraska State Immunization Information System (NESIIS). You can also call the Information Line, 402-444-3400, available 8:30 a.m. to 4 p.m. Monday through Saturday. If you are having trouble accessing your vaccination records through NESIIS, please consult your healthcare provider for assistance. NESIIS also has a Help Line at 888-433-2510 or can be reached by email at firstname.lastname@example.org (Note -- If you were vaccinated outside of Nebraska, you’ll need to contact that state or vaccine provider).
Are winners responsible for any taxes? What about impact on scholarships?
Winners will be responsible for any federal, state and local taxes associated with the drawings. All federal, state and local laws and regulations apply. Winners are also responsible for determining how prizes may impact any scholarships and financial aid such winner currently receives.
Before accepting any prize, validated and verified winners should consult with their own tax advisors to understand the potential tax impact to them of accepting and receiving the prize. Additionally, validated and verified winners should consult with financial aid to determine the potential impact on scholarships or financial aid currently received.
How many times can I enter the drawing?
Only one (1) entry into the drawing is permitted throughout the duration of the drawing.
How do you plan to keep my vaccination information private?
The data system housing the vaccination registry is designed with the highest standards of encryption and security protocols for maintaining data classified with this level of sensitivity. Full information is visible to members of the UNL Public Health Advocacy Team who will verify submitted information. These team members are trained in information privacy, security and confidentiality practices. Aggregate, de-identified data will be utilized by members of the Public Health Advocacy Team for the purposes of understanding the breadth of vaccination rates in our community in order to more accurately prepare for Fall. Only your name, contact information, and status as having submitted verified vaccination information will be used in the selection and distribution of prizes.
Can I decline the prize to remain anonymous?Yes. If you prefer to keep your vaccination status and participation in the registry confidential, if you are selected a winner you may choose to decline the prize.
This drawing (this “Drawing”) is open only to Fully Vaccinated Individuals (hereinafter defined) who are either (i) Eligible UNL Students, as further defined below, that are 19 years of age and older as of July 1, 2021; or (ii) Eligible UNL Faculty or Staff, as further defined below, that are 19 years of age and older as of July 1, 2021.
The term “Fully Vaccinated Individual” means a person that has received the full-required dosage of a COVID-19 vaccine approved by the U.S Food and Drug Administration, European Union, or World Health Organization and recognized by the University of Nebraska–Lincoln (“UNL”). UNL currently recognizes the following COVID-19 vaccines: AstraZeneca – SK Bio, Beijing Bio-Institute of Biological Products Co. – Sinopharm, Johnson & Johnson/Janssen, Moderna, Pfizer/BionTech, Serum Institute of India - Covishield, and Sinovac Life Sciences Co. UNL reserves the right to, at any time and without notice, add and remove COVID-19 vaccines from its list of recognized vaccines.
The term “Eligible UNL Student” means any person that is (i) admitted and enrolled as a degree-seeking at UNL for the Fall 2021 academic semester; (ii) is in good standing at the University (iii) a Fully Vaccinated Individual.
The term “Eligible UNL Faculty or Staff” means any person that is (i) currently employed by UNL and receives a paycheck in a permanent, temporary, full-time, or part-time position; (ii) not on a disciplinary sanction under UNL’s employment policies and regulations at the time of entry; and (iii) a Fully Vaccinated Individual. Eligible UNL Faculty or Staff specifically excludes: (i) members of the Chancellor’s Executive Leadership Team and the COVID-19 Task Force; (ii) members of the senior administrative team; (iii) members of a senior administrative person’s immediate family members; and (iv) household members of a senior administrative person. “Immediate family members” shall mean parents, stepparents, children, stepchildren, siblings, stepsiblings, or any of their spouses. “Household members” shall mean people who share the same residence at least three (3) months in one (1) calendar year.
The sponsor (the “Sponsor”) of this Drawing is the University of Nebraska–Lincoln, located at 1400 R Street, Lincoln, NE 68588.
How To Enter:
Eligible UNL Students and Eligible UNL Faculty and Staff may enter this Drawing by submitting their vaccine documentation, including, but not limited to, vaccination cards issued by the Center for Disease Control or records issued by medical providers, to the Vaccine Registry Portal by 11:59:59 p.m. (all times Central Standard Time) the day prior to a drawing, the schedule of which is listed below (each, a “Drawing Date” and collectively, “Drawing Dates”). Any entries received after 11:59:59 p.m. the day before a Drawing Date will not be considered for prizes given on that certain Drawing Date, regardless of whether the late receipt resulted from human, computer or transmission error, or other causes, but such entries will be considered for all other Drawing Dates. UNL reserves the right to (i) determine, in its sole and absolute discretion, qualifying vaccine documentation; and (ii) verify any and all submissions to the Vaccine Registry Portal against any available governmental immunization databases.
UNL will conduct drawings on each of the following dates in 2021: (i) July 9; (ii) July 16; (iii) July 23; (iv) July 30; (v) August 6; and (vi) August 13 (collectively, the “Regular Drawings”). A final grand prize drawing will be conducted on or after August 15, 2021 (the “Final Drawing”).
By entering and participating in this Drawing, all participants agree that: (i) no purchase is necessary to enter or win; (ii) participation in this Drawing is entirely voluntary; and (iii) COVID-19 vaccination is not required in order to attend or be employed by, as applicable, UNL.
An individual may only enter this Drawing one (1) time throughout the duration of this Drawing. Any individual found using multiple entries will be disqualified. All qualified entries, including winning entries, present on a Drawing Date will be carried through to all other Drawing Dates.
Seven (7) Eligible UNL Students and one (1) Eligible UNL Faculty or Staff member will be selected as weekly winners from all qualified entries on a Drawing Date. Five (5) Eligible UNL Students and one (1) Eligible Faculty or Staff member are permitted to win the grand prizes described below. Winners will be selected based on the criteria set forth in these Official Rules.
Winning Eligible UNL Students during the Regular Drawings will be eligible to receive the following: (i) meal plan of $250 Dining Dollars; (ii) Husker items valued at $250; (iii) one (1) academic semester of four (4) 60-minute massages from Campus Recreation valued at $240; (iv) one (1) four-month (4-month) all-access pass for unlimited climbing at Campus Recreation upon completion of a Climbing Basics Class or passage of a Skills Check prior to climbing and a semester-long group fitness pass and locker rental, and three (3) personal training sessions valued at $176; (v) one (1) smart watch valued at up to $350; (vi) free weekly ice cream at the Dairy Store for two (2) people for one (1) academic year valued at $260; and (vii) membership to Nebraska Innovation Studio Makerspace for one (1) year valued at $180. Winning Eligible UNL Students during the Final Drawing will receive one (1) year of resident undergraduate tuition and fees valued at $9,872 which can be awarded in the form of cash or scholarship.
Winning Eligible UNL Faculty or Staff during the Regular Drawings will be eligible to receive the following; (i) one (1) reserved and named parking spot for one (1) calendar year valued at $1,056; (ii) Lied Center season tickets for two (2) to the Broadway series valued at $632; (iii) 2021 Husker football season tickets for two (2) valued at $784; (iv) 2021 Husker volleyball season tickets for two (2) valued at $504; (v) one (1) 60-minute massage at Campus Recreation each month for an academic semester valued at $240; and (vi) Champions Club season pass for two (2) for the Husker football 2021 season valued at $140. The winning Eligible UNL Faculty of Staff during the Final Drawing will receive a trip to Dublin, Ireland to attend the August 2022 Husker football game against Northwestern with Chancellor Green and Mrs. Green, valued at $8,000, which includes (i) a roundtrip flight in economy for two (2) from Omaha, Nebraska to Dublin, Ireland; (ii) hotel costs for two (2) nights; and (iii) two (2) tickets to the Husker football game in Dublin, Ireland; however, all other incidental expenses, including, but not limited to, transportation, meals, and all other costs and expenses not specifically listed as part of a prize in these Official Rules and which may be associated with the award, acceptance, receipt, and use of all or any portion of the prize are solely the responsibility of the winner. The Sponsor is not responsible if the August 2022 Dublin, Ireland Husker football game against Northwestern University is cancelled, postponed or delayed for any reason whatsoever, and the Sponsor will not be obligated to award a replacement prize.
All federal, state and local taxes associated with the receipt or use of any prize are the sole responsibility of the winner and UNL encourages winner to consult with financial aid or a tax advisor prior to accepting such prize. A winner may choose to decline the prize, and in such event the Sponsor will award the prize to the next qualified entry based on the selection criteria.
Winners during this Drawing will be randomly selected from among all eligible entries received. Each winning entry selected will be subject to verification of eligibility and compliance with these Official Rules. UNL reserves the right to determine in its sole and absolute discretion whether winners meet eligibility and compliance requirements. Upon UNL’s satisfactory conclusion of each winner’s verification of eligibility and compliance with these Official Rules, such winners will be declared the official winners of this Drawing.
Each winner will be notified by the Sponsor via email at the address provided by the winner in his or her entry. Such notification will include instructions on how to claim the prize. To claim the prize, a winner must follow the instructions in the notification. Prizes must be claimed within three (3) business days of the notification. If the individual does not claim the prize within this time, the Sponsor will award the prize to the next qualified entry based on the selection criteria. The Sponsor will use all reasonable efforts to award all prizes within six (6) weeks of notification of the award.
All prizes are non-transferable. No cash or other substitutions are allowed. Decisions of the Sponsor are final. Each individual’s odds of winning are dependent on the overall number of entries submitted during this Drawing.
In the event a winner does not meet these Official Rules or eligibility requirements as determined by the Sponsor, he or she forfeits the prize. In such event, the Sponsor will award the prize to the next qualified entry based on the selection criteria. This process will continue until the prize is awarded.
Safety, Security, and University Policies:
By entering this Drawing and/or accepting the use of the prize, each winner agrees that the Sponsor may, at its option, conduct a background check on each such winner. If the Sponsor chooses to conduct a background check, a separate authorization may be required from each winner and his or her guest as determined by the Sponsor in its sole and absolute discretion. Each winner agrees to complete any such additional authorization or otherwise forfeit the prize. The Sponsor reserves the right to disqualify any potential winner from receiving any prize based on such background check if Sponsor determines, in its sole discretion, that awarding any prize to such winner could result in a safety or security risk to any person or persons or could result in the disruption of any event associated with this Drawing. In the event a winner is disqualified, the Sponsor will award the prize to the next qualified entry based on the selection criteria.
By entering this Drawing and/or accepting the use of the prize, each winner and their guests, as applicable, agree to all policies, rules, and regulations of the Sponsor and its affiliates related to the use of the Prize.
By submitting an entry for this Drawing, entrants agree to abide by these Official Rules and any decision the Sponsor makes regarding this Drawing and, if selected as a winner, to cause their guests to abide by these Official Rules, as applicable. To the extent that these Official Rules conflict with any information provided for this Drawing, these Official Rules control.
Each entrant and any guest of a winner releases the Sponsor and its subsidiaries, and affiliates, and each of their directors, officers, employees, and agents from any and all liability for any injuries, losses, or damages of any kind directly or indirectly arising from or relating to this Drawing, any prize, any acceptance, possession, or use of any prize, or presence on, near or within the University of Nebraska–Lincoln, or the field, or participation in any activity therein, and waives any and all causes of action, related to any claims, costs, injuries, losses or damages of any kind arising out of or in connection with the content or delivery, misdelivery, acceptance, possession, use of or inability to use any prize (including, without limitation, claims, costs, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light, whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory.
This promotion and the rights and obligations of the Sponsor, entrants, winner, and guests of winners will be governed and controlled by the laws of the State of Nebraska, applicable to contracts made and performed therein without reference to the applicable choice of law provisions. All actions, proceedings and litigation relating hereto must be instituted and prosecuted solely within the State of Nebraska, Lancaster County. The Sponsor, entrants, winner, and guests of winners consent to the exclusive jurisdiction of the state courts of Nebraska and federal courts located within the state and county listed above with respect to any action, dispute or other matter directly or indirectly pertaining to or arising out of this promotion, any prize, any acceptance, possession or use of any prize, presence on, near or within the University of Nebraska–Lincoln, or participation in any activity therein. Entrants, winners, and guests of winners in this Drawing hereby waive any right to jury trial relating to this Drawing against the Sponsor. With respect to any claim against the Sponsor relating to this Drawing or any prize, any acceptance, possession or use of any prize, valid service of process and proper filing of any claim in a court of competent jurisdiction must occur within three (3) months of the date such claim accrued, or such claim shall be forever waived, barred and discharged.
By entering this Drawing and/or accepting the use of the prize, each entrant and any guest of a winner agree to permit (except where prohibited by law) the Sponsor to use his or her photo, video, audio, image, likeness, name, current city, hometown and prize information, without limitation and in any format, for purposes of advertising, promotion and publicity for the Sponsor without compensation or further permission. By entering this Drawing, each entrant agrees to hold harmless Sponsor and its subsidiaries, and affiliates, and each of their directors, officers, employees, and agents from any claims or expenses arising as a result of any allegation that the entrant did not own or was not authorized to allow publication and reproduction of the content included in a post. By entering this Drawing and/or accepting the use of the prize, each entrant and any guest of a winner agree to hold harmless Sponsor and its subsidiaries, and affiliates, and each of their directors, officers, employees, and agents from any claims or expenses arising as a result of any negligence or willful misconduct of such entrant or guest of a winner.
To the extent allowed by applicable law, all entries submitted as part of this Drawing are the property of the Sponsor. Each entrant agrees that the Sponsor shall be the exclusive owner of all right, title and interest in and to such entries and may use such entries for any purpose without compensation.
The Sponsor reserves the right to modify or change the drawing rules, or cancel, suspend or reschedule this Drawing, at any time in its sole discretion. If for any reason this Drawing is not capable of being conducted as planned due to any cause beyond the control of the Sponsor, including but not limited to any event or cause which corrupts or affects the administration, security, fairness, integrity or proper conduct of this Drawing, the Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend this Drawing and disqualify any individual who may have caused any such problem. The Sponsor assumes no responsibility for any theft or destruction, unauthorized access to or alteration of entries. The Sponsor is not responsible for misdirected, lost, late, or damaged entries, busy phone signals, unreturned voicemail messages, unreturned email messages, email notifications caught in SPAM filters or other forms of computer protection software or incomplete entries. The Sponsor is not responsible for any problem or technical malfunctions of any telephone network or lines, computer online systems, servers, computer equipment, software, failure of any entry to be received by the Sponsor on account of technical problems or delays, including any injury or damage to an entrant’s or any other person’s computer or mobile device related to or resulting from participation or downloading any materials in this Drawing.
The Sponsor reserves the right to disqualify from this Drawing, and to prosecute, and seek damages from, to the fullest extent permitted by law, any entrant or winner who, in the Sponsor’s reasonable suspicion, tampers with the entry process, attempts to deliberately damage any application or web site or undermine the legitimate operation of this Drawing, violates these Official Rules or acts in an unsportsmanlike or disruptive manner.
By entering this Drawing, each entrant agrees that (i) any and all disputes, claims, and causes of action arising out of or in connection with this Drawing or the prize awarded, shall be resolved individually, without resort to any form of class action; (ii) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs, directly or indirectly associated with entering this Drawing, but in no event will attorneys’ fees be awarded or recoverable; and (iii) under no circumstances will any entrant be permitted to obtain any award for, and entrant hereby knowingly and expressly waives all rights to, any damages other than actual out-of-pocket expenses.
Each entrant and any guest of a winner shall execute and deliver such additional documents, instruments, conveyances and assurances and take such further actions as may be required to carry out the provisions of these Official Rules.
The invalidity or unenforceability of any provision of these Official Rules shall not affect the validity or enforceability of any other provision. In the event that any provision is determined to be invalid or otherwise unenforceable or illegal, these Official Rules shall otherwise remain in effect and shall be construed in accordance with their terms as if the invalid or illegal provision were not contained herein. In no event shall the sponsor be liable for any consequential, special, indirect, incidental, or punitive damages, even if the sponsor has been advised of the possibility of the damages. In any even the maximum aggregate liability of the sponsor to any person directly or indirectly relating to this drawing or the award, use or or enjoyment of any prize shall not exceed the sum of $50.00.
